Community First Services & Supports (CFSS) & PCA
At Golden Heart Home Health Services, we provide compassionate, hands-on personal support to help seniors, adults, and children with disabilities remain safe and independent at home. As the first CFSS Certified agency in Crow Wing County, we are fully equipped to guide families through Minnesota's transition from traditional PCA to CFSS.
Whether you need traditional PCA support or are transitioning into the expanded flexibility of CFSS, our caregivers work under custom plans designed around your daily preferences.
Key Support Areas:

Activities of Daily Living (ADLs)
Direct assistance with bathing and grooming, dressing and undressing, skin care, bowel and bladder care, and eating.

Mobility & Physical Health
Support with exercise, ambulation, transfers, and positioning to maintain safety and movement.

Health-Related Support
Dependable medication reminders and assistance with routine daily health tasks.

Household Assistance (IADLs)
Help with essential laundry, light housekeeping, and maintaining a safe living space.

Behavioral Support & Monitoring
Redirection and intervention for behaviors, including continuous observation and active safety monitoring.

Homemaking & Household Support Services
A clean, safe, and comfortable home environment is essential to maintaining overall health and independence. Our Homemaking Services support seniors, individuals with disabilities, and those recovering from an illness with daily household tasks, nutritious meal care, and personal companionship.
We handle everyday household chores and routine management so clients can focus on their comfort and wellbeing.
Comprehensive Homemaking Support Included:

Light Housekeeping: Routine cleaning to maintain a safe, orderly living space, including dusting, sweeping, vacuuming, and making beds.

Laundry & Clothing Care: Washing, drying, folding, ironing clothes, and changing bed linens.

Meal Preparation & Clean-Up: Planning, cooking nutritious meals, washing dishes, and maintaining a clean kitchen.

Companionship & Engagement: Meaningful social interaction, reading together, conversing, and playing cards or board games.

Social & Household Support: Assisting recovering, elderly, or disabled clients with light administrative needs, phone calls, and daily routine management.
Respite Care & Night Supervision
Night Supervision provides overnight assistance and supervision in a person’s own home when there is an assessed need for overnight support. Depending on the individual’s assessed needs and service authorization, support may be provided by awake or asleep staff.
Respite services provide temporary support to individuals while offering relief to family members or primary caregivers. Services are based on each person’s assessed needs, eligibility, and authorized service plan.

Individual Community Living Supports (ICLS) & IHS
Individual Community Living Supports (ICLS) and Individualized Home Supports (IHS) are designed to empower individuals with disabilities or age-related challenges to build self-sufficiency.
We work one-on-one with clients to develop, maintain, and improve the skills necessary to live independently and stay actively engaged in their local communities.

Veterans (VA) Home Care & Private Pay Services
Golden Heart proudly honors military veterans by providing specialized in-home care services tailored to their service-related needs and VA benefits. We assist honorably discharged veterans and surviving spouses in accessing quality care right in their homes.
For individuals or families who do not qualify for state waiver programs or prefer direct care arrangements, our Private Pay options offer customized scheduling without government program restrictions.
